Depression and Medical Certificate

Assalamualaikum,

I am a medical doctor. One of the first things that I was taught when I first started doing my housemanship (doktor pelatih) back in 2002 was how to fill a medical certificate (MC), its indication, the rules and regulation, the responsibilities a prescribing doctor carries with it as well as the medical ethics behind it. It is not as easy as A,B and C.

Back then, a houseman was still allowed to write an MC. Nowadays, they have lost the privilege. You need to complete your 2-year housemanship and become a medical officer to enable you to write an MC. A doctor needs to be ensure that the patient is really sick (and not pretending to be sick) and the sickness prohibits him from working because it affects his performance, he needs time to recover and the disease is contagious.

The duration of MC depends on the illness and the most likely duration needed for the patient to gets better or the illness to heal. For example, upper respiratory tract infection (cold/flu) may take up to 2-3 days to get getter, a laceration wound may take a week, a fracture bone can take up to 3 months...so it varies. The doctor's judgment must be based on his knowledge and experience and according to the medical ethics.

I was surprised to read the paper today exposing the duration of MC given to husband of PAS candidate for DUN Tenang, Maarof Abdul Mutalib. This information was first mentioned in marahku.blogspot.com. The blog also shows the certificates if your want to view them. The total MC were 21 days. The first MC was given by a private clinic (Pusat Perubatan Keluarga) in Labis (6-7 January 2011). The rest of the MCs were produced by Klinik Kesihatan bandar Putra, Segamat (10-23, and 24-28 January 2011).

Now, let us look at these figures. The 28th of January is today (Friday), tomorrow is Saturday, which is already a public holiday and the election day is on Sunday (30/01/2011). Hmmm....really, something fishy is going on here. According to the statement given by the PAS candidate herself (Puan Normala Sudirman) the MC was given because the husband suffered from Depression (Kemurungan). She said anybody who questions the MC should see and ask the doctor who had given out the MC himself.

I think the Ministry of Health should investigate this matter further by taking statements from the doctors who wrote the MC. It is not difficult to trace because every doctor has to write down their full name, IC number as well as their registration number on the MC. We know that a private clinic can only give a maximum of 2-day MC, but for the government hospitals and clinics, the doctors can give more.

Questions:
1. Why the MC has to be 3 weeks (21 days)? Why until today?  Just before the election?

2. Who is the doctor who given out the MC? Does he/she has the same political view as the candicate's husband party and hence want to help the party indirectly by giving the MC (in other words, not being honest and not following a doctor's oath and ethics)?

3. What criteria the doctor uses to diagnose depression? There are a number of criteria needed to fulfill the diagnosis of depression based on DSM IV (which is used by Psychiatrists all over the world) . I list them here from a website (in lay people's terms)

According to DSM-IV, the diagnostic manual from American Psychiatric Association, criteria used by mental health professionals, you have Major Depressive Disorder if:

bullet
You have had an episode of depression lasting at least two weeks with at least five of the following symptoms:

(1) You are depressed, sad, blue, tearful.
(2) You have lost interest or pleasure in things you previously liked to do.
(3) Your appetite is much less or much greater than usual and you have lost or gained weight.
(4) You have a lot of trouble sleeping or sleep too much.
(5) You are so agitated, restless, or slowed down that others have begun to notice.
(6) You are tired and have no energy.
(7) You feel worthless or excessively guilty about things you have done or not done.
(8) You have trouble concentrating, thinking clearly, or making decisions.
(9) You feel you would be better off dead or have thoughts about killing yourself.

bullet
These symptoms are severe enough to upset your daily routine, or to seriously impair your work, or to interfere with your relationships.
bullet
The depression does not have a specific cause like alcohol, drugs, medication side effect, or physical illness.
bullet
Your depression is not just a normal reaction to the death of a loved one.

You see... you need at least 5 of the above criteria PLUS an episode of depression lasting for 2 weeks. I don't think he suffers from this illness if we based on the above criteria. He was really happy, smiling and up and about from what I can see on the Buletin Awani news at 7pm today. However, I might be wrong because I don't see him myself, take a thorough history and full examination.

4. He is a teacher at SMK Tenang Stesen. How can he is on MC and then go out, campaigning with his wife all over the DUN Tenang area? Is this a right thing to do?

5. If he really suffered from a depression, shouldn't he be on anti-depressive medication? If he needs 3 weeks of MC, would he be better to be admitted to hospital for treatment? Psychiatric ward maybe?

6. Was he ever seen by a psychiatrist (pakar sakit jiwa)? He needs proper treatment if he really suffers from depression.

I think  the Ministry of Health needs to investigate this matter further and interrogate the medical doctors involved. I really think something fishy is going on here...If he really is sick, he needs treatment and proper attention from a qualified psychiatrist. I he does not, then the doctors had done something terribly wrong, sinful and misuse his position as a doctor, do something against his oath, medical ethics and code of conducts and actions should be taken.

PAS, PAS....I really don't expect this from PAS people (who are supposed to be religious, honest (amanah), responsible towards themselves/other people (if you are sick, please seek treatment, take appropriate medication and then, rest at home - not campaigning. If you are not, then, you should go to school and teaches your students, that is your amanah and responsibility).

I gave PAS credit in my previous post. This time, sorry PAS. I am so frustrated with you.

Enough said.

Solat dhuha

AlhamdulilLah...

Seorang sahabat ku mengajak aku utk solat dhuha kejap tadi,

"Sudah". Jawab ku, pendek.

Dia tercengang..."power". Balasnya pendek. "Aku pun nak kaya jugak" tambahnya lagi.

Aku tersengeh. Aku tau dia suka duit (sape tak suka duit???). Aku baru bagitau dia masa breakfast tadi, kalau nak rezeki bertambah, kena tambah amalan sunat, solat dhuha, solat taubat, istighfar....Dia tahu, rezeki aku makin bertambah murah lepas buat bisnes Score A ni...

Mari kita lihat hadith berkenaan solat dhuha:

Al-Bukhari:
Narrated Abu Huraira: My friend (the Prophet) advised me to do three things and I shall not leave them till I die, these are: To fast three days every month, to offer the duha prayer, and to offer Witr before sleeping.  (Book #21, Hadith #274)

Sunan Abu Dawood:
Narrated AbuUmamah: The Apostle of Allah (peace_be_upon_him) said: If anyone goes out from his house after performing ablution for saying the prescribed prayer in congregation (in the mosque), his reward will be like that of one who goes for hajj pilgrimage after wearing ihram (robe worn by the hajj pilgrims). And he who goes out to say the mid-morning (duha) prayer, and takes the trouble for this purpose, will take the reward like that of a person who performs umrah. And a prayer followed by a prayer with no worldly talk during the gap between them will be recorded in Illiyyun.  (Book #2, Hadith #0558)

Rakan2, mari sama2 kita tambah amalan kita, insyaAllah.

Shake hand

Assalamualaikum,

Narrated Qatada: I asked Anas, "Was it a custom of the companions of the Prophet to shake hands with one another?" He said, "Yes."  (Book #74, Hadith #279)

Kalau tau Haram pun kadang2 dihentamnya jugak. Jadinya, bersalaman lelaki -perempuan yang bukan mahram ni dah jadi macam lumrah, nauzubilLah.

Kalau orang-orang politik (terutamanya Melayu UMNO yang pangkat2 menteri tu) tak payah cakap la, terang2 pun bersalam, siaran langsung pun bersalam, takde rasa bersalah langsung. Kadang2 pelik, nak kata tak sembahyang, sembahyang, nak kata tak tau...takkkan la tak tau...dah tua bangka pun masih buat dosa...pasal aurat tak payah cakap la....next entry la.

Tak payah la kita sibuk nak menyalahkan orang lain, salahkan diri sendiri dulu. Cuba cermin diri sendiri dulu. hari ni apa aku dah buat satu hari. Kalau mati hari ni, rasa2nya, ke Syurga ke, ke Neraka? Haaa, jawab sendiri.

Marilah sama2 kita membaiki diri masing2.

Meninggal Dunia Ketika Lepas Solat Asar

Lapan hari berlalu. Ustaz M dah mula nak melupakan pasal jenazah imam ini.

Tapi, dengan takdir Allah SWT, hari ke-9, telefon bimbit ustaz M berbunyi.

"Nampak macam nombor hospital nie", getus hati ustaz M.

"Hello, ustaz M ye?"... satu suara bertanya.

"Ya, ustaz M bercakap".

"Ustaz ingat lagi tak mayat imam yang meninggal masa solat Asar tempohari?", sang suara meminta kepastian.

"Imam...imam. ...yanng mana satu....ermm. .oh...ya. .ya..ingat. .ingat... ", ustaz M memberi kepastian dalam ingat-ingat lupanya.

"Dokumen dah siap, boleh ustaz datang sepital, uruskan tak?", beritahu sang suara ringkas.

"Ok..ok..., sekarang juga aku bergerak, setengah jam lagi aku sampai! ",
balas ustaz M.

Lega hati ustaz M. Dapat juga dia uruskan mayat tersebut setelah beberapa hari 'pending'. Ingatkan dah tak jadi apa-apa dah. Lama bebenor pihak hospital nak mengurus kan dokumennya.

..Seolah- olah imam ni baru aje meninggal sebentar tadi. Mayatnya tidak keras dan tidak pula membeku! Macam orang sedang tidur. Ustaz M kelu lidahnya. Begitu juga orang di sekeliling.

Istimewa sungguh mayat ini. Kalau daging mayat biasa dah lama mengeras dan membeku. Masya-Allah!

MENINGGAL KETIKA BACA KHUTBAH JUMAAT

......... ......... .". Tiba-tiba imam ini terdiam....

Semua para jemaah diam (memang dengar khutbah kena diam pun) dan hanya memerhati sahaja ke arah imam. Semua mata tertumpu kepadanya.

Imam masih lagi senyap. Matanya terpejam rapat. Tidak bergerak. Suasana hening seketika.

Ya,

Wangi sungguh! Tak pernah ustaz M mencium bau wangian sebegini rupa selama dia menguruskan jenazah lain sebelum ini! Hebat mayat ni, fikir ustaz M...

Subhanallah!

Hati ustaz M begitu melonjak girang untuk meneruskan memandikan jenazah imam sehingga selesai. Hati kecilnya berdoa ;

"Ya Allah! Engkau Maha Mulia! Engkau telah memuliakan mayat ini, maka
muliakanlah ia sehingga aku selesai memandikannya. Aku begitu seronok
melihat jenazah dalam berkeaadan begini. Ya Allah...Ya Allah..Ya
Allah....... ......"

Doa
ustaz M dimakbul Allah SWT.

Thank you everyone!

Assalamualaikum,

I would like to thank all the surgeons, doctors, staff nurses and all the staffs involved during the admission-surgery-and ward care of my untie in Universiti Kebangsaan Malaysia Medical Centre (UKMMC) from 16-19 january 2011. Well done. May Allah rewards all of you, ameen.

The surgery was carried out on the 17th of January 2011 in OT8 of the general operation theatre (GOT). She was given general anaesthesia (GA) by Dr. Wong (anaesthetist), Dr. Neoh (registrar) and Dr. Komala (registrar). The main surgeon was Mr. Farshid and he was assisted by 2 other surgeons, Mr. Yazid Kassim and Ms. Hazla. The excision of the tumour was uneventful. The whole surgery took approximately an hour. The tumour was sent for histopathology examination (HPE).

Surgeons in action - Ms Hazla and Mr Farshid (and Mr Yazid Kasim behind Ms Hazla)

Incision over the Right Scapular

The excised tumour (im plastic bag)

The recovery process was smooth. The staff nurses in female Orthopaedic ward were marvelous. She was discharged with oral antibiotic and analgesics. Just now, she demostrated to me her newly-gained, painless range of motion (ROM) of the right shoulder. It was impressive. It is back to normal again. Now, all that I need to do is daily wound dressing at home (I am well-trained to do this hehehe, alhamdulilLah) until it completely heals.

I am so grateful. AlhamdulilLah. All praise to Allah. Thank you again everyone.

DAP dah mula tunjuk belang sebenar

Assalamualaikum,

Dah terang lagi bersuluh. Nak berkawan lagi dengan DAP.

Dah la kilang arak Carlsberg masih tegak berdiri.

Apa hadis nabi Muhammad SAW bersabda:
Narrated Abdullah ibn Umar: The Prophet (peace_be_upon_him) said: Allah has cursed wine, its drinker, its server, its seller, its buyer, its presser, the one for whom it is pressed, the one who conveys it, and the one to whom it is conveyed.

Nak kena translate ke?

Konon2 nya berjaya mendakwahkan DAP la, sebab pemimpin dan pemidato bukan Islam mereka boleh sebut beberapa patah/ayat al-Quran...kat depan dia sebut la ayat Quran tu sbb nak mengelirukan ahli2 PAS/PKR yang kononnya diaorg ni tertarik dengan Islam.... kat belakang, macam2 usaha yang mereka buat. Apa makna arahan Ronnie Liu ni? kalau tak tahu, mai saya nak bagitau.

Baik la kita bekerjasama sesama Islam (semua orang Melayu beragama Islam bukan? Baca sini utk definisi Melayu). Bekerjasama tak bermaksud kena ada dalam 1 parti, ia bermaksud bersatu dalam memperjuangkan hak berkaitan kepentingan Islam - baik politik, ekonomi, pendidikan mahupun sosial. Kalau DAP-MCA-Gerakan boleh duduk semeja semasa membincangkan kepentingan hal ehwal perniagaan kaum Cina, kenapa tidak kaum Melayu yang beragama Islam ni?

Why I hate Israel

http://www.almanar.com.lb/NewsSite/NewsDetails.aspx?id=170004&language=en

****************************************************************************

Salam,

You may ask me why I put the 'Boycott Israel' logo as my signature thumbnail. This recent news is one of million reasons that I can give. Israel illegal occupation on Palestinian land has long history. You can read the story, the struggle, plot and meticulous strategy and planning by the early zionist entities to create the state of Israel - and they succeeded.

Since then, the hardship of the Palestinian began. The tears and blood. The the Muslim world - watch closely....and watch....and watch...and see....and see.....Oh! I See (OIC) can only see until now...

Who to blame?

The Jews? The Christians? The Arabs?

I think we should blame ourselves first before pointing our fingers to others. The jews (Zionis) had planned 100 years ahead of us in shaping the world order. And we are struggling to be united even in a stand against a common enemy. We cooperate with the enemy to kill own brothers-in-Islam, to destroy and impose sanction on neighbouring Muslim state, to seek medical treatment in their countries (and put your heart/life in their hand), do you believe in them more than your own Muslim brothers?(I am referring to the Saudi King recent visit to USA). I think you do.

Waahh...I am talking at state level, maybe that's too much for you. Maybe, we should just look at ourselves first then (to talk at personal level, and to be more realistic and easier to ponder...)

Why don't we answer these few simple questions first before we talk big:

  1. When was the last time I pray in congregation in the mosque (excluding friday prayer)?
  2. Do I perform non obligatory prayer? Solat sunat - rawaatib, dhuha, witir. Are my prayer perfect enough to gain ticket to the heaven? You can go to Bukit Jalil to buy ticket to a football match, but not for this one bro...
  3. When was the last time I make non-obligatory fasting?
  4. When was the last time I visit a sick Muslim brothers/sisters-in-Islam
  5. How many Istighfar a day do I say? Tasbih? Tahmid? Tahlil?
  6. Do I make du'a everytime I perform prayer? or I just leave immediately after salam? Do I understand the meaning of the du'a?
  7. Do I understand the meaning of Al-Fatihah? How old are you now? How many times have your perform salat and read al-Fatihah (at least 17 times per day) for how many years now and you can't even at least memorize the meaning of al-Fatihah? You can memorise lyrics of many songs right? how many songs? Siti Norhaliza songs? Faizal Tahir songs? Awie? Search? Wings?
  8. Do I struggle financially to meet my needs every month? Very low income huh? Why don't you change it then?
  9. Who is my idol? A singer? A western singer? Actors? Actress? Fasha Sandha is it? or Pierre Andre? Rosham Noor? Can they give you syafaat (help and blessings) in the hereafter?
  10. Do I prefer to go to a musical concert, or to a nasyeed performance? Francisca Peters vs Maher Zain? Wings Vs Raihan?
  11. Do I prefer to watch a movie at the cinema. or to go to Islamic forum? Anugerah Juara Lagu (AJL)? - Is it entertaining? Can bring you to the Jannah is it?
  12. Do I watch 'Forum Perdana' in the TV when it is on? or do I change it to different channel?
  13. Does my wife wear veil (tudung)? What about my sister? My mother? My cousin? Do I care?
  14. Do I take care of my aurat (body parts that are forbidden for others to see)?, Or do I prefer others to see them? Or do I care about it at all? Is my shirt and pants tight? Do I expose my forearms? Does my veil (tudung) cover my chest? Do I wear stocking? Is't your feet and forearms are aurah as well?

Bla...bla...bla...Do you ask yourselves these questions??? You ask yourself and I ask my self. You know yourselves better than other people. Am I asking questions that are too much to ask? Am I a fundamentalist? Islamic fundamentalist? If you said, Islamic fundamentalists are those who want to stick to the fundamentals of Islam - then, I am.

Don't talk big, don't talk about uniting the people, the country or the Muslim world if you can't even do simple thing... to change your own self. Israel can do whatsoever it wants for the next 100 years, there is nothing for them to worry, as long as WE ARE LIKE WE ARE at the current state.

Enough said. This is what I think.

The Story of Appreciation

Salam.

A very close friend of mine forwarded this story to me. I want to share it with you too. Hopefully it will give us insight into our daily life.

Here goes:-

One young academically excellent person went to apply for a managerial position in a big company.

He passed the first interview, the director did the last interview, made the last decision.

The director discovered from the CV that the youth's academic achievements were excellent all the way, from the secondary school until the postgraduate research, never had a year when he did not score.

The director asked, "Did you obtain any scholarships in school?"
The youth answered "none".

The director asked, " Was it your father who paid for your school fees?"
The youth answered, "My father passed away when I was one year old, it was my mother who paid for my school fees.

The director asked, " Where did your mother work?"
The youth answered, "My mother worked as clothes cleaner.

The director requested the youth
to show his hands. The youth showed a pair of hands that were smooth and perfect.

The director asked, " Have you ever helped your mother wash the clothes before?"
The youth answered, "Never, my mother always wanted me to study and read more books. Furthermore, my mother can wash clothes faster than me.

The director said, "I have a request. When you go back today, go and clean your mother's hands, and then see me tomorrow morning.*

The youth felt that his chance of landing the job was high. When he went back, he happily requested his mother to let him clean her hands. His mother felt strange, happy but with mixed feelings, she showed her hands to the kid.

The youth cleaned his mother's hands slowly. His tear fell as he did that. It was the first time he noticed that his mother's hands were so wrinkled, and there were so many bruises in her hands. Some bruises were so painful that his mother shivered when they were cleaned with water.

This was the first time the youth realized that it was this pair of hands that washed the clothes everyday to enable him to pay the school fee. The bruises in the mother's hands were the price that the mother had to pay for his graduation, academic excellence and his future.

After finishing the cleaning of his mother hands, the youth quietly washed all the remaining clothes for his mother.

That night, mother and son talked for a very long time.

Next morning, the youth went to the director's office.

The Director noticed the tears in the youth's eyes, asked: " Can you
tell me what have you done and learned yesterday in your house?"

The youth answered, " I cleaned my mother's hand, and also finished
cleaning all the remaining clothes'

The Director asked, " please tell me your feelings."

The youth said
  • Number 1, I know now what is appreciation. Without my mother, there would not the successful me today.
  • Number 2, by working together and helping my mother, only I now realize how difficult and tough it is to get something done.
  • Number 3, I have come to appreciate the importance and value of family relationship.

The director said, " This is what I am looking for to be my manager. I want to recruit a person who can appreciate the help of others, a person who knows the sufferings of others to get things done, and a person who would not put money as his only goal in life. You are hired."

Later on, this young person worked very hard, and received the respect of his subordinates. Every employee worked diligently and as a team. The company's performance improved tremendously.

A child, who has been protected and habitually given whatever he wanted, would develop "entitlement mentality" and would always put himself first. He would be ignorant of his parent's efforts. When he starts work, he assumes that every person must listen to him, and when he becomes a manager, he would never know the sufferings of his employees and would always blame others.

For this kind of people, who may be good academically, may be successful for a while, but eventually would not feel sense of achievement. He will grumble and be full of hatred and fight for more. If we are this kind of protective parents, are we really
showing love or are we destroying the kid instead?*

You can let your kid live in a big house, eat a good meal, learn piano, watch a big screen TV. But when you are cutting grass, please let them experience it. After a meal, let them wash their plates and bowls together with their brothers and sisters. It is not because you do not have money to hire a maid, but it is because you want to love them in a right way. You want them to understand, no matter how rich their parents are, one day their hair will grow gray, same as the mother of that young person. The most important thing is your kid learns how to appreciate
the effort and experience the difficulty and learns the ability to work with others to get things done.

Let us share this story with as many as people as possible...this may change somebody's fate!...

A rewarding journey up north...

Assalamualaikum,
Yesterday was an exhilarating day for me....
Unbelievable...

0630 Woke up from sleep. Prayed Fajr prayer. Packed some bread and beverage for breakfast.

0730 Fetched Lenie and Azwan from their apartment in Taman Midah and headed north
towards Sabak Bernam. Had breakfast while driving

0945 Reached Kolej Komuniti Sabak Bernam. Nobody was there...(sigh). Supposed to start at
10am. Waited till 11am. 3 participants came accompanied by my sister-in-law (Intan)
and her hubby (Mokhlis).

1130 I began the session with 'Al-Fatihah'. 2 speakers - Dr. Shahnaz talked about the Score A
programme, an excellent online education portal for Malaysian students followed by Dr.
Lenie who explained thoroughly regarding the marketing plan for those interested to
make this programme as a business. Intan and Mokhlis also gave their heart-touching sharing regarding the programme. Congrats!! The session ended at 1245 noon.

1315 Continued the journey up north. Had some bread again for lunch.... Then, we had trouble
finding our way out to the North-South Highway Exit...luckily Azwan brought his GPS
along. Fuhh...what a relief!

1445 Reached North-South Highway. Drove up north...so hungry but could not afford to stop
as the session in Alor Setar was supposed to start at 1630 (4.30pm). We got the news
form Dr. Nini, who is our leader in this programme that 30 people attended the session
in Penang this morning! Great news! Hopefully we got an excellent turn-out too in Alor
Setar yeay!

1730 Reached Alor Setar. Had problem to find Regent Hotel (the hotel was not shown on GPS
hahaha). After 3 phone calls asking the hotel staff, finally we arrived to the hotel. No
wonder it was not in the GPS, it was a small, and old 3-storey hotel. But, the interior was
not too bad, no prayer room though...and guess what? Only a friend was there, waiting
for us...(sigh). We took it positively, doesn't matter!! This is only a challenge...

1800 We prayed Zuhur/Asar prayer in the hall corner while waiting for other participants to
come. Yet, nobody came except 6 persons who already joined the programme. We
changed the session as a small group meeting (SGM) instead. Started the session at 1830
(6.30pm)

2000 We prayed maghrib/Isha. Had an excellent dinner at PakDa Restaurant, Alor Setar.
Thanks Lenie for the great dinner (that was our lunch too actually). We ate rice, tomyam,
vegetable soup and 'sotong celup tepung'...burrppp!!

2200 Drove further up to Perlis. Reached Dr. Lenie's home...met her mother. Took shower,
refreshed myself. Had a good boost of Nescafeeee....

2345 Begin the journey back home.

0430 Dropped Lenie and Azwan at their Apartment.

0500 Reached home, finally. AlhamdulilLah...Thank Allah for the safe journey, Now, I can rest
my back on my bed....

The journey took almost 24 hours...from KL-Sabak Bernam-Alor Setar-Perlis-KL. Unimaginable initially that I can do that. The aim was just to spread the knowledge about Score A, assist our children to excel in education and at the same time to share the opportunity for those who wants to make this Score A as a business, find extra income, give regular money to our parents especially in the challenging world we're living right now...

I will continue this struggle...insyaAllah. Those who are interested can email me. You can also read more on this programme here. Our company info is here.
